Long Term Grants Reporting Requirements

GHHS members that receive a long-term grant are expected to submit three reports for their project to be considered completed satisfactorily and to receive full funding. 

Interim Report:
The first report, outlining your project and progress, is to be submitted one year after receiving grant funds. If we do not receive your interim report by then, GHHS reserves the right to retain the remaining funds not issued. The report is typically one page single-spaced summarizing your experience, progress on the project and plans for the future. You must submit a report to GHHS to be approved for the second payment (equivalent to one third of the total awarded amount) of your long-term grant. 

Final Report:
At the conclusion of your project, grant awardees must submit a final report for their project to be considered completed satisfactorily. You must submit a report to GHHS to be approved for the third payment of your grant funding. Grant awardees will submit a 250-500-word summary of the project, a monetary breakdown of spending, and any resources, tools, and photos utilized during project implementation that may assist other GHHS members with replicating a similar project.

Dynamic Report Guidelines: In addition to the final report, we are requesting a dynamic report in the format of your choosing (podcast recording, Instagram live, webinar, presentation, etc.) to share with the GHHS community. Project leaders are expected to coordinate with GHHS staff to ensure that the dynamic report is executed within the 24-month period of receiving grant funding.
